PTLDS often includes severe fatigue, cognitive dysfunction (referred to as "brain fog," memory, and concentration difficulties), musculoskeletal pain, headaches or migraines, vertigo or dizziness, migrating pains in joints or tendons, sleep disturbances such as insomnia, numbness or tingling in limbs, and rare neurological experiences like feeling "out of body." These symptoms may persist despite the absence of detectable active infection after antibiotic treatment, potentially due to lingering inflammation or persistent infection in some cases.
Treatments for PTLDS currently focus on symptom management as there is no established treatment to completely cure PTLDS. Standard antibiotic therapy is generally not extended beyond recommended courses, as long-term antibiotics have not shown consistent benefit. Supportive care aimed at managing specific symptoms like pain relief, cognitive rehabilitation, and addressing sleep disturbances is also crucial. Some clinical programs, such as the Aviv Medical Program, claim to help patients return to active lifestyles, but these approaches are still being studied.
Early detection and prompt antibiotic treatment of initial Lyme infection reduce the risk of developing PTLDS. Delays in diagnosis and treatment correlate with higher likelihood and severity of persistent symptoms.
