In the lush, green landscapes of summer, foragers often come across mushrooms, some edible and others potentially deadly. One such deadly mushroom is the death cap, a deceptively harmless-looking fungi that can cause severe poisoning with a high mortality rate.

The death cap mushroom, scientifically known as Amanita phalloides, is notoriously difficult to identify, even for experts. It resembles edible mushrooms such as champignons, russulas, and milk caps, making it a dangerous imposter in the wild.

Proper preparation before consumption does not neutralize the poison in some cases for any mushrooms. The death cap mushroom contains the toxic substance alpha-amanitin, with a lethal dose of 0.1 micrograms per kilogram of body weight. This toxin is stronger than cobra venom and is deadly; there is no antidote.

Death cap mushroom poisoning initially appears as severe gastroenteritis delayed by hours after eating. The first signs include headache, vision disturbances, vomiting, and severe abdominal pain. After a brief symptom-free period of about a day, serious organ failure follows, primarily affecting the liver and kidneys, which can lead to coma and death if untreated.

Symptoms may improve or disappear temporarily during the latent phase, but the amatoxins continue to damage internal organs silently. In the subsequent days, liver failure, kidney dysfunction, and multi-organ failure may occur, often leading to fatal consequences without prompt specialized care.

If mushroom poisoning is suspected, immediate medical attention is critical. Contact poison control centres or emergency services promptly. While waiting for the ambulance, the victim should rinse their stomach with water, a laxative, potassium permanganate, or activated charcoal.

Despite all modern achievements in medicine, the mortality rate from severe death cap poisoning is as high as 90%. The department head of extracorporeal detoxification and hemodialysis at the City Clinical Hospital of Emergency Medical Care in Grodno warns against picking or consuming the death cap mushroom.

Foragers must exercise extreme caution when identifying mushrooms. The death cap mushroom has a white, brownish, or gray-green appearance, with gills under the cap that are also white. A distinctive feature is the white ring at the top of the stem, which may be barely noticeable. The torn cup-shaped volva at the bottom of the death cap's stem should also be noted.

Cases of death cap poisoning, including fatal ones, are almost annually recorded in Belarus. With the growing popularity of foraging, it is essential to spread awareness about the dangers of the death cap mushroom and encourage caution when identifying and consuming wild mushrooms.
