Craniotomy in the retrosigmoid region: Aim and surgical outcomes explanation
A retrosigmoid craniotomy is a surgical procedure that offers a minimally invasive approach to treating a variety of conditions in the brain. This technique, favoured for its reduced brain retraction compared to other craniotomies, provides effective access to tumors, vascular malformations, and cranial nerves in specific regions of the brain.
The retrosigmoid craniotomy offers a clear view of and access to the cerebellum, the brain stem, the facial nerve (cranial nerve VII), the vestibulocochlear nerve (cranial nerve VIII), and certain blood vessels. This makes it particularly useful for lesions in the cerebellopontine angle and posterior fossa, such as acoustic neuromas (vestibular schwannomas), meningiomas, skull base tumors, trigeminal neuralgia, colloid cysts, and arteriovenous malformations (AVMs).
Acoustic neuromas, non-cancerous tumors from Schwann cells on the vestibular nerve, are commonly removed using this approach. Meningiomas, tumors originating in the meninges, can also be accessed and removed with this craniotomy. The retrosigmoid intradural suprameatal approach allows for preservation of critical cranial nerves during resection, making it ideal for skull base and petroclival tumors.
Trigeminal neuralgia, a condition that causes severe facial nerve pain, is treated with microvascular decompression via a retrosigmoid approach. Colloid cysts, cysts that can occur centrally in the brain, and AVMs, abnormal clusters of blood vessels, can also be treated using this technique.
During the procedure, a person will be under general anesthesia. In some cases, a local anesthetic may be administered instead, especially when the person needs to be awake during the procedure, such as in facial nerve decompression cases. After the procedure, people usually wake up soon after it's complete, although in some cases, a surgeon may keep the person asleep for longer to allow recovery.
Recovery from a retrosigmoid craniotomy typically involves a hospital stay of up to 10 days, followed by a full recovery period of around 6-12 weeks. During this time, people will not be able to perform certain activities such as driving, participating in contact sports or strenuous physical activity, traveling by air, and will attend follow-up appointments. It is normal to feel some soreness or discomfort after the procedure, and feeling tired after waking up from general anesthesia is common.
While major complications following a craniotomy are not common, they can include infection, swelling, scarring, vasospasm, cerebrospinal fluid leak, bleeding, brain injury, brain swelling, brain hemorrhage, neurological deficits, speech difficulties, damage to facial nerves, damage to sinuses, memory problems, blood clots, hearing loss, balance and coordination problems, headaches or migraine, stroke, coma, and stroke.
The outlook for someone after a retrosigmoid craniotomy will depend on many factors, such as the reason for their procedure and how effective the surgery was. Many people fully recover and go on to resume their typical activities after having a retrosigmoid craniotomy.
A study comparing outcomes after tumor resection in different areas of the brain found that those with cerebellopontine angle tumors had the fewest complications postsurgery. Before the procedure, a person's doctor will explain the procedure, take a medical history, run tests, and discuss medications. On the day of the procedure, a person will not be allowed to eat or drink anything, and will need to follow additional instructions from their healthcare team.
